Mahaba Community Services LLC was established to address the critical need for comprehensive housing stabilization services in Minnesota. We recognized that stable housing is fundamental to individual and family well-being, and that many people need specialized support to achieve and maintain housing stability.
Our approach is rooted in understanding that each person's housing journey is unique, influenced by their circumstances, culture, and individual needs. We provide person-centered services that respect dignity, promote self-determination, and build on individual strengths.
As a Medical Assistance enrolled provider, we ensure our services meet the highest standards while remaining accessible to those who need them most. Our team is committed to continuous learning and improvement to better serve our community.

We provide housing stabilization services throughout the state of Minnesota. Our services are designed to meet the diverse needs of Minnesota communities while complying with all state regulations and Medical Assistance requirements.
Primary Service Area: Statewide Minnesota coverage
Service Limitations: Services cannot be provided outside of Minnesota
Transportation: Available with client present for housing-related discussions
To be eligible for our housing stabilization services:
- Must have active Medical Assistance coverage
- Must be living in their own home (for sustaining services)
- Cannot be concurrently receiving certain other services (ACT, housing transition)
- Must meet HCBS settings rule requirements if renting
- Services cannot be provided in provider-owned/controlled settings
As a Medical Assistance enrolled provider, we maintain strict compliance with all applicable regulations:
- Minnesota Department of Human Services requirements
- Home and Community-Based Services (HCBS) settings rule
- Federal and state privacy and confidentiality laws
- Documentation and reporting standards
- Service limitation and authorization requirements
